I don't think it was too flukey, look at the early exits made at the previous World Cup by the 'big' teams at the hands of the minows.
A better idea would simply to play 2 matches a day, perhaps a morning and afternoon start. It seems part of the reason to avoid the associates being involved is to cut down on the length but the length can be cut down that way, you don't get many casual watchers of the group games so revenues should hardly be affected in that regard.

BCLG's option sounds ok and is pretty normal in footbal as well where the group stages have several matches each day. For cricket to expand beyond the traditional countries that seems a good solution. When the cricket WC started it got a fair bit of attention in the dutch media and I'm sure that's only because our national team participated.
