I don't really mind being 1st or 2nd, up to Beam !!
There should be some barbarians, with some camps by the seaside, but I think normal barbarians setting is fine. As for the starting island, it should be at least a 1st core of cities (so that's like 5 to 10), maybe a bit more, but don't feel forced to put our capital in the middle, and keep some surprises in this area for us !
