OK let me try and get this down right...

Version 1.14

1) Make a map in the editor.
2) Set custom player properties.
3) In the player properties screen set all players on the difficulty level you want.
4) Do all the other bits you wanna.
4) Save the scenario in the scenario folder of Civ3PTW.
5) Start PTW.
6) Go to the MP screen.
7) Host a local game.
8) Set the difficulty level for the one you want.
9) Make sure AccProd is switched off and respawn is off.
10) Choose PBEM or Hotseat accordingly.
11) From the Game type, choose load scenario.
12) Pick your scenario.
13) Fiddle with the player numbers on the left hand side of the players.
14) Click rename, and rename the first player as the player.
15) Launch game.
16) Set ADMIN password and remember it. This will be sent to the first player so that they can make their own password.
17) Set player password. this can be anything.
18) Save the game.
19) Quit the game.
20) Go to the MP screen.
21) Host a local game.
22) Set the difficulty level for the one you want.
23) Make sure AccProd is switched off and respawn is off.
24) Choose PBEM or Hotseat accordingly.
25) From the Game type, choose load game.
26) Pick the game you just saved.
27) Fiddle with the player numbers on the left hand side of the players.
28) Launch game.
29) Save Immediately. This save will be sent to the first player.
30) Play on a bit to make sure that the difficulty level is correct and that Acc Prod isn't set on.
31) Done.

Version 1.21
1) As above but you make the scenario and then start a PBEM with it.
2) Set passwords as above.
3) Save the game and just send to the first player.
4) I think they have fixed the problems in PBEM levels.

Meli posted this at CFC some time ago - I just printed it out then and have used it ever since.

I'm not 100% certain you actually NEED to load once as a scenario then once as a saved game but it works. I get a lot of crashes along the way though with ptw locking at the point when you launch the game - maybe 50% of the time
